Device Requests

Faculty and Staff can request the following Accessories: Webcam / Headset/ USB Adapter/ Voice Amplifier  |  (One Per user) 

Submit the self-service request using the online request form below.  Once approved, you will receive an approval email with instructions on where to pick-up your devices and you will have 1 week from approval date to pick up the item(s).

 

Note: This request form is not for assets such as laptops/ipads/upgrades/etc.  For those requests you will need to contact your Department IT Coordinator (DITC).  Full-time faculty or MPPs requiring a laptop will need to contact their DITC to submit a request. 

Note for Department IT Coordinators:  This is a self-service form to be filled out by the faculty/staff requesting the item.  Do not submit this form on behalf of your faculty/staff.

CAMPUS DEVICE PICKUP (FACULTY/STAFF)

Monday - Friday, 8am - 5pm
Saturday - Sunday, CLOSED
(Check Academic Calendar for campus closures)

Location: PLS - 48A, Pollak Library South basement, by the loading docks

Device pickup is for approved requests only. Please have your TitanCard or Photo ID with you.
