CAMPUS Re-entry UPDATES
Verification of Employee Vaccine Records
Prior to Chancellor Castro's announcement to implement a COVID-19 vaccination requirement upon FDA approval for the CSU system, our campus developed a plan for safe re-entry which includes required weekly testing for all students and employees. State and ASI employees that choose to be vaccinated now can opt out of the testing plan by submitting thier vaccination record for verification online through a third-party vendor, Point and Click Solutions (PNC), that is HIPAA compliant. Information will be sent soon to ASC employees outlining how they can upload their vaccination records. Learn more.
Instructions for Submitting Your Vaccine Record
COVID-19 Vaccination Clinics
In partnership with St. Jude Medical Center – CSUF employees, students, and their family may visit St. Jude’s On-Site Vaccination Clinic to obtain the COVID-19 vaccine. Appointments are available until May 28, be sure to sign up prior to arrival. Other vaccine clinics are listed on the Titans Return: Vaccine webpage .
Campus Re-entry Update
Employees should continue to prepare to return to work on campus in the summer, most likely by July 1. MPP supervisors will give employees advance notice of their specific return to work date. In anticipation of this safe return to campus, the temporary telecommute agreements will end for all employees on June 30. Read more.
Changes to Single Day Access Requests

Effective now, employees coming to campus for COVID-19 testing no longer need to complete a Single Day Access Request form.

Effective Thursday, July 1, staff, managers, and faculty who are on a 12-month contract and coming to campus no longer need to complete a Single Day Access Request form.

Effective Monday, August 2, faculty on academic year contracts and coming to campus no longer need to complete a Single Day Access Request form.
Fall Planning Forums for Employees
HRDI and Academic Affairs will continue to partner and host Fall Planning Forums in the summer months for all employees. These one-hour forums will include the most up-to-date information about health and safety guidelines and our planning for the future. EAP Workplace Re-entry Support Sessions
As the campus plans for re-entry, a variety of Employee Assistance Program (EAP) group sessions are available to employees to share and discuss thoughts while gaining tips and strategies for transitioning back to work. Visit the HRDI Coronavirus webpage for more details and updates.

Paperless Forms Launch – Payroll Services
As we enter the third phase of the HRDI paperless forms launch, all Payroll Services eForms in MyWorkplace will transition to the new Adobe Experience Manager (AEM) portal on Friday, May 21. Visit the HRDI Forms page for access to the forms, instructions on how to navigate AEM.

Second Annual One Book, One CSUF Program
Titans, you voted, and the book you selected for the 2021-2022 academic year is: They Called Us Enemy, by George Takei. Stay tuned for opportunities to receive a free copy of the book in the fall. In the meantime, if you’d like to start reading They Called Us Enemy over the summer, the book is available at the CSUF Pollak Library . In addition, the Orange County Public Library and Los Angeles Public Library have physical and electronic copies available for check out.

COVID-19 Self-Reporting Form
The University requests that any employee who tests positive for COVID-19 or becomes aware that they may have been in close contact with someone who either has tested positive for or is suspected to have COVID-19 report the positive result or exposure using the CSUF COVID-19 Self-Reporting Form .
