PART 2: Gather Letters of Recommendation and Recommendation forms from three people and send them to the DNP PROGRAM

Each applicant must submit THREE Letters of Recommendation with forms. You can download the recommendation form herePDF File . We will accept the forms several different ways:

Email *Preferred Method*: Your evaluators can send the completed form and a letter of recommendation via email to dnp@fullerton.edu. 

Mail directly: The recommender can mail the form and letter directly to:

Applicant mails: It is also acceptable to return the form and letter to you *in a sealed envelope* and you can submit them to DNP at the mailing address. Please understand that these letters and forms are confidential and must be in an envelope sealed by the recommender.

 

Part 3: SUBMIT 1 Official Copy of Each transcript to the DNP Program

You can submit your official transcripts in one of the following formats: 

Electronic *Preferred Method*: If the service asks for an email address, the transcript must be sent to admissions@fullerton.edu.

IMPORTANT: Open transcripts are NOT accepted for admissions purposes. Transcripts must be in the original, unopen, sealed  envelope.
