Application Procedures
Fall 2020 Application
Priority Deadline: December 15, 2019
Final Application Deadline: March 29, 2020
The Fall 2020 MPH application cycle opens on October 1, 2019. All applicants must complete the online application found on Cal State Apply . The four quadrants under "My Application" must be completed: Personal Information, Academic History, Supporting Information, and Program Materials.
The priority application deadline is December 15, 2019 and the final application deadline is March 29, 2020. Because the MPH Program's incoming class each year is approximately 20-25 students, we strongly encourage applicants to submit their application and supporting materials before the priority deadline to receive full consideration. Applicants who submit before the priority deadline will also be notified of their application status earlier.
Step I - Complete the first three quadrants on Cal State Apply .
- Personal Information
- Academic History (applicants may bypass "Transcripts Entry" section only)
- Supporting Information (include ALL information regarding your Employment and your Achievements and Awards)
Step II - Upload the items listed under CSUF MPH Program Materials.
- Current resume or curriculum vitae
- Statement of purpose/interest letter. Provide 300-500 words describing how the program relates to your professional goals.
Note: the Statement of Purpose under "Supporting Information" is general and will be sent to ALL CSU schools/programs you apply to.
- Unofficial GRE scores. Reminder: Submit official scores to CSUF (see Step IV).
- Unofficial transcript(s). Reminder: Submit official transcripts to CSUF (see Step IV).
Step III - Indicate your recommenders under CSUF MPH Program Materials.
- Two (2) letters of recommendation. One must be academic, the second can be academic or professional. Recommenders will need to answer a set of questions and include their narrative.
Note: Our recommendations are now online. Each recommender you identify will receive an email from Cal State Apply with a link to the portal. Each recommender must create an account to utilize the system. We strongly suggest that you notify your recommenders of all required activities ahead of time.
Step IV - Submit official materials to CSUF.
- Submit official GRE score(s) to the following: School Code: 4589; Department Code: 0616.
Note: Applicants are encouraged to take the GRE General Test at least two weeks prior to the application deadlines.
- Submit official transcript(s) from each campus attended. Please have your college send the official transcript to the following address:

Note: If previously accepted as an undergraduate at CSUF, only transcript(s) from any school attended after acceptance is required. Students currently in graduate status at CSUF do not need to reapply; instead, submit a “Change of Objective” request form (available at LH-114) to Admissions and Records and complete steps below.
